P\u00ebr shumic\u00ebn e njer\u00ebzve, marrja e nj\u00eb qet\u00ebsuesi t\u00eb thjesht\u00eb dhimbjeje \u00ebsht\u00eb e mjaftueshme p\u00ebr t\u00eb &quot;liruar&quot; dhimbjen e kok\u00ebs. Megjithat\u00eb, ekziston nj\u00eb p\u00ebrqindje mjaft e madhe e bashk\u00ebqytetar\u00ebve tan\u00eb q\u00eb vuajn\u00eb ose nga dhimbje koke shum\u00eb intensive ose t\u00eb shpeshta ose nga t\u00eb dyja. Nga hulumtimet dihet se mbi 20% e popullat\u00ebs s\u00eb p\u00ebrgjithshme dhe ndoshta mbi nj\u00eb e treta e t\u00eb gjitha grave vuajn\u00eb nga dhimbje koke kronike q\u00eb k\u00ebrkojn\u00eb trajtim t\u00eb ve\u00e7ant\u00eb. Format m\u00eb t\u00eb r\u00ebnd\u00ebsishme t\u00eb nj\u00eb dhimbje koke t\u00eb till\u00eb jan\u00eb migrena dhe dhimbje koke e tipit tension. Sondazhet e m\u00ebdha nga Anglia dhe Danimarka tregojn\u00eb se 14-151 TP3T e popullat\u00ebs s\u00eb p\u00ebrgjithshme mungojn\u00eb n\u00eb pun\u00eb brenda nj\u00eb viti p\u00ebr shkak t\u00eb k\u00ebtyre dhimbjeve t\u00eb kok\u00ebs. N\u00eb SHBA b\u00ebhen 10 milion\u00eb vizita te mjeku n\u00eb vit p\u00ebr shkak t\u00eb dhimbjes s\u00eb kok\u00ebs.<\/p>\n<p>Megjithat\u00eb, relativisht pak pacient\u00eb marrin trajtimin e duhur p\u00ebr dhimbjen e tyre. Ato (kryesisht femrat) q\u00eb vuajn\u00eb nga migrena jan\u00eb pacient\u00ebt m\u00eb t\u00eb keqtrajtuar dhe m\u00eb t\u00eb pak\u00ebnaqur. P\u00ebr k\u00ebt\u00eb arsye ata mund t\u00eb p\u00ebrdorin &quot;forma alternative t\u00eb trajtimit&quot; q\u00eb premtojn\u00eb mrekulli, jan\u00eb t\u00eb pabazuara n\u00eb m\u00ebnyr\u00eb shkencore dhe potencialisht t\u00eb rrezikshme. \u00cbsht\u00eb vler\u00ebsuar se rreth gjysma e pacient\u00ebve me migren\u00eb<br \/>\nata do t\u00eb zhg\u00ebnjehen me trajtimet e gabuara dhe do t\u00eb heqin dor\u00eb dhe nuk do t\u00eb k\u00ebrkojn\u00eb nj\u00eb kur\u00eb t\u00eb provuar shkenc\u00ebrisht, ata thjesht do t\u00eb durojn\u00eb n\u00eb m\u00ebnyr\u00eb fataliste migren\u00ebn e tyre duke g\u00eblltitur ila\u00e7et kund\u00ebr dhimbjeve nga nj\u00eb grusht.<\/p>\n<p>Nga ana tjet\u00ebr, trajtimi korrekt i dhimbjes s\u00eb kok\u00ebs k\u00ebrkon fillimisht klasifikimin e sakt\u00eb t\u00eb saj \u2013 di\u00e7ka jo e leht\u00eb as p\u00ebr neurolog\u00ebt me p\u00ebrvoj\u00eb. Klasifikimi m\u00eb i fundit nd\u00ebrkomb\u00ebtar \u00ebsht\u00eb i vitit 2018 dhe \u00ebsht\u00eb nj\u00eb lib\u00ebr i t\u00ebr\u00eb me disa kategori dhe n\u00ebnkategori dhimbje koke.<br \/>\n\u00cbsht\u00eb thelb\u00ebsore q\u00eb mjeku t\u00eb sqaroj\u00eb n\u00ebse b\u00ebhet fjal\u00eb p\u00ebr nj\u00eb dhimbje koke par\u00ebsore (p.sh. migren\u00eb, dhimbje koke t\u00eb tipit tension) apo dyt\u00ebsore (p.sh. nga nj\u00eb infeksion). M\u00eb pas ai do t\u00eb vendos\u00eb p\u00ebr trajtimin e duhur (p.sh. simptomatik ose parandalues). M\u00eb posht\u00eb jan\u00eb renditur kategorit\u00eb kryesore t\u00eb dhimbjeve primare t\u00eb kok\u00ebs. Migrena \u00ebsht\u00eb nj\u00eb form\u00eb e zakonshme e dhimbjes s\u00eb kok\u00ebs. Shum\u00eb studime epidemiologjike kan\u00eb treguar ndikimin (barr\u00ebn) e madhe negative t\u00eb gjendjes tek individi, n\u00eb familjen dhe jet\u00ebn e tij shoq\u00ebrore si dhe n\u00eb sistemin sh\u00ebndet\u00ebsor.<\/p>\n<p>Migrena mund t\u00eb jet\u00eb me ose pa aur\u00eb (zakonisht). Aura \u00ebsht\u00eb zakonisht n\u00eb fush\u00ebn e shikimit t\u00eb pacientit dhe i paraprin dhimbjes s\u00eb fort\u00eb. Ai zgjat disa minuta zakonisht deri n\u00eb nj\u00eb or\u00eb dhe mund t\u00eb ket\u00eb gjithashtu mpirje n\u00eb duar ose k\u00ebmb\u00eb. Dhimbja e migren\u00ebs \u00ebsht\u00eb e lokalizuar n\u00eb gjysm\u00ebn e kafk\u00ebs, \u00ebsht\u00eb shum\u00eb intensive dhe pulsuese ose godit\u00ebse si nj\u00eb \u00e7eki\u00e7. Pacientja zakonisht shmang drit\u00ebn dhe zhurmat dhe ka t\u00eb p\u00ebrziera dhe t\u00eb vjella t\u00eb cilat mund ta leht\u00ebsojn\u00eb at\u00eb. Zgjat, pa trajtim, nga 4 or\u00eb deri n\u00eb 3 dit\u00eb. Zakonisht lidhet me periodat e nj\u00eb gruaje, me ndryshimet e motit, me ushqime t\u00eb caktuara.<\/p>\n<p>Nuk ka sh\u00ebrim p\u00ebr migren\u00ebn, POR trajtimi i duhur mund t\u00eb zvog\u00ebloj\u00eb si frekuenc\u00ebn ashtu edhe intensitetin e dhimbjes.<br \/>\nnga ngjarjet vaskulare dhe p\u00ebrmir\u00ebsojn\u00eb shum\u00eb cil\u00ebsin\u00eb e jet\u00ebs s\u00eb pacientit. Nj\u00eb studim i madh i koh\u00ebve t\u00eb fundit tregoi se ve\u00e7an\u00ebrisht grat\u00eb me migren\u00eb kan\u00eb nj\u00eb shans t\u00eb shtuar p\u00ebr infarkt miokardi ose goditje n\u00eb tru. Megjithat\u00eb, p\u00ebrqindja n\u00eb p\u00ebrgjith\u00ebsi p\u00ebr femrat e reja \u00ebsht\u00eb shum\u00eb e ul\u00ebt dhe nuk ka arsye p\u00ebr panik, k\u00ebto femra thjesht duhet t\u00eb marrin trajtim t\u00eb specializuar p\u00ebr migren\u00ebn e tyre.<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<h3>TENSIONI DHIMBJE KOKE<\/h3>\n<p>\u00cbsht\u00eb gjithashtu nj\u00eb form\u00eb shum\u00eb e zakonshme e dhimbjes s\u00eb kok\u00ebs (ndoshta m\u00eb e zakonshme se migrena). Forma episodike e k\u00ebsaj dhimbje koke mund t\u00eb zgjas\u00eb deri n\u00eb nj\u00eb jav\u00eb dhe dhimbja \u00ebsht\u00eb zakonisht n\u00eb t\u00eb gjith\u00eb kafk\u00ebn, kryesisht n\u00eb ball\u00eb me nj\u00eb ndjesi kur edhe syt\u00eb \u201cr\u00ebndohen\u201d. Dhimbja ndihet m\u00eb shum\u00eb si nj\u00eb pesh\u00eb ose &quot;shtr\u00ebngim&quot; si nj\u00eb unaz\u00eb, nuk \u00ebsht\u00eb aq intensive sa n\u00eb migren\u00eb dhe mund t\u00eb filloj\u00eb n\u00eb qaf\u00eb. Zakonisht nuk ka t\u00eb p\u00ebrziera ose t\u00eb vjella dhe drita nuk \u00ebsht\u00eb ve\u00e7an\u00ebrisht shqet\u00ebsuese. Zakonisht pacient\u00ebt jan\u00eb njer\u00ebz mjaft t\u00eb shqet\u00ebsuar. \u00cbsht\u00eb nj\u00eb dhimbje koke q\u00eb edhe pse e shpesht\u00eb, \u201ce neglizhohet\u201d. Ka mjekim simptomatik dhe parandalues p\u00ebr dhimbjen e kok\u00ebs nga tensioni, si dhe m\u00ebnyra t\u00eb ndryshme p\u00ebr t\u00eb \u201crelaksuar\u201d tensionin si p.sh. Dhimbje koke grupore. \u00cbsht\u00eb nj\u00eb form\u00eb mjaft e rrall\u00eb e dhimbjes s\u00eb kok\u00ebs par\u00ebsore, por shum\u00eb e dhimbshme. \u00cbsht\u00eb pak m\u00eb e zakonshme tek meshkujt dhe \u00ebsht\u00eb nj\u00eb dhimbje koke q\u00eb duket se vjen n\u00eb t\u00eb nj\u00ebjt\u00ebn koh\u00eb \u00e7do dit\u00eb. Zgjat nga 15 minuta deri n\u00eb 3 or\u00eb, \u00ebsht\u00eb shum\u00eb intensiv n\u00eb gjysm\u00ebn e fytyr\u00ebs, rreth syrit, i cili loton dhe skuqet, si dhe fryrje n\u00eb an\u00ebn e kund\u00ebrt dhe vrim\u00ebn e hund\u00ebs. Pacienti zakonisht ka disa episode t\u00eb tilla n\u00eb dit\u00eb (2 deri n\u00eb 3 deri n\u00eb 8 n\u00eb dit\u00eb). Pacienti \u00ebsht\u00eb shum\u00eb i shqet\u00ebsuar dhe l\u00ebviz n\u00ebp\u00ebr dhom\u00eb.<\/p>\n<p>Dhe n\u00eb k\u00ebt\u00eb dhimbje koke t\u00eb v\u00ebshtir\u00eb dhe t\u00eb dhimbshme ka mjekim p\u00ebr periudhat kohore (zakonisht 1-2 muaj) q\u00eb ndodh.<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<h4>MJEK\u00cbSIA P\u00cbR DHIMBJEN E KOK\u00cbS<\/h4>\n<p>P\u00ebr t\u00eb gjitha arsyet q\u00eb p\u00ebrmend\u00ebm (v\u00ebshtir\u00ebsi n\u00eb dallimin e dhimbjes s\u00eb kok\u00ebs, trajtim i ve\u00e7ant\u00eb p\u00ebr forma t\u00eb r\u00ebnda, t\u00eb rralla ose t\u00eb rrezikshme<br \/>\ndhimbje koke) jan\u00eb formuar n\u00eb mbar\u00eb bot\u00ebn n\u00eb spitale apo klinika private t\u00eb dhimbjes s\u00eb kok\u00ebs q\u00eb merren me trajtimin e bazuar shkenc\u00ebrisht t\u00eb k\u00ebtyre pacient\u00ebve.
